At ACTIVATECR, our dedicated team, including nephrologists, psychologists, and cross-trained therapists, has extensive experience treating patients with CKD and ESRD. We have witnessed firsthand the important role mental health services play in improving patient outcomes by helping them overcome the feelings of hopelessness that can lead to missed dialysis treatments, non-compliance with medication and treatment plans, and a lack of self-care.

Unfortunately, access to mental health services often involves long wait times, leaving patients without the support they need. Our mission is to bridge this gap by providing specialized mental health services tailored to each CKD or dialysis patient’s needs within weeks instead of months. Our web-based referral system streamlines the process, making it quick and easy for social workers and medical providers to refer patients. We take it from there, scheduling appointments and providing convenient telehealth mental health services while ensuring CMS compliance through appropriate follow-through for patients identified as having mental health concerns.
